They needed to ensure consistent data across both platforms, allowing clinicians to view and update patient information (such as medications and procedures) in real time. The solution utilized HL7 FHIR standards for interoperability. Technically, it involved deploying an on-premises middleware layer (FHIR transformation/orchestration) to facilitate bidirectional data exchange with the master EHR. A cloud backend was established on AWS using API Gateway and Lambda, incorporating Cognito for robust authentication and authorization. Furthermore, identity federation was implemented using common Identity Providers (IdPs) like Okta or Azure AD, enabling Single Sign-On (SSO) across both on-premises applications and cloud services.
