This research paper tackles the significant interoperability challenges arising from fundamental differences in data modeling approaches between openEHR and HL7 FHIR. To address this, the authors developed FHIRconnect, a novel domain-specific language (DSL) and an open-source transformation engine designed to enable standardized, bidirectional data exchange. The system utilizes a triple-layered architecture, aiming to resolve interoperability gaps by achieving mapping reuse based on international archetypes. Specifically, it successfully mapped 24 international archetypes to 15 FHIR profiles across seven clinical domains. This work provides a comprehensive DSL and an open-source execution engine (openFHIR), establishing a technical foundation that reduces reliance on custom ETL solutions and advances both syntactic and semantic interoperability in healthcare IT systems built on open standards.
