This application allows healthcare systems using major EHRs—including Epic, Cerner, and Allscripts—to embed VectorCare's full patient logistics management capabilities directly within their existing workflows. The goal is to transform how hospitals coordinate discharges and post-acute care at scale. The app enables case managers to access the VectorCare Hub (a no-code workflow builder) directly inside the EHR. Care teams can request, schedule, and monitor essential services—such as ambulance transport, home health services, DME, and rides via Uber or Lyft—all without switching applications. Furthermore, users can view real-time updates, including estimated arrival times and live GPS tracking, right within the patient's EHR record. This seamless integration coordinates the entire continuum of care (e.g., scheduling a ride home, arranging oxygen delivery, booking a home health visit) from within the EHR. This unified experience provides critical operational benefits: accelerating discharge and bed turnover, reducing emergency department boarding times, and lowering readmission risk.
