Health Information Systems (HIS) are foundational for digital health ecosystems, but their complexity often leads to fragmentation and poor interoperability across multiple stakeholders. This systematic review analyzed literature from 2020–2025 to assess the application of key architectural patterns—such as service-oriented, microservice, and decentralized ledger architectures—and their alignment with ecosystem requirements. Findings indicate that while microservices dominate current HIS designs, FHIR-based contracts are crucial for stabilizing interfaces and reducing integration costs. However, significant gaps remain in areas like cross-border care, sustainability, and AI integration, emphasizing the need for greater architectural diversity and intersectoral collaboration to build resilient digital health infrastructures.
