Global FHIR Adoption Trends: Analysis of Key Use Cases and Challenges
Which FHIR Use Cases Are Gaining Momentum Globally? - Healthcare
HL7 announced it is working on FHIR standards following a global survey of FHIR use cases.
Summary
HL7 released global survey results from 2025, revealing key drivers and barriers to FHIR adoption. Diagnostic orders and reports emerged as the leading use case, with broad adoption by EHR vendors and care providers. However, significant challenges persist, including lack of knowledge and unclear benefits.

Details
HL7 partnered with Firely in 2025 to conduct a global online survey analyzing the current state of FHIR adoption. Responses from 52 countries highlighted key use cases such as prescriptions/pharmacy, terminology, and diagnostic orders/reports. Notably, 'diagnostic orders and reports' was identified as rapidly advancing, surpassing document exchange and referrals. The survey found that the main drivers for FHIR adoption are 'Innovation' and 'Regulation and grants,' with many countries mandating or advising the use of standards in electronic health data exchange. However, the biggest challenge cited is a 'lack of FHIR knowledge,' alongside concerns about 'unclear benefits' and 'high investment cost.' HL7 plans to develop new accelerators, such as Gizmo for medical device interoperability, and is considering an accelerator for quality measures. Furthermore, HL7 reported aiming to build networks through the CMS interoperability pledge, allowing patients access to all electronic and medical information via applications of their choice.
