Germany stands out as a global benchmark for 'profile reuse' due to its high number of packages and dependencies. This success is attributed to strong national coordination and clear rules. In contrast, the US shows activity but faces fragmentation due to its market-driven system, leading to an increase in custom profiles that challenge consistency. Canada similarly struggles with national interoperability because of significant provincial autonomy. These findings suggest that not only the adoption of technology but also a country's governance model and coordination structure (e.g., Netherlands' NL Core) critically impact FHIR implementation and spread. Establishing standardized core profiles and mechanisms to promote their reuse is crucial for countries, including Japan.
