Regenstrief Institute has released LOINC version 2.81, a major update to the Logical Observation Identifiers Names and Codes (LOINC), a global standard for health terminology created in 1994. The latest release introduces two new linguistic variants: Arabic for Jordan and Czech for the Czech Republic, bringing the total number of supported languages to 22. Beyond language expansion, the update includes over 4,000 new and updated concepts. Key technical enhancements include introducing a new LABORDERS.ONTOLOGY class that provides nearly 2,000 new lab test ordering concepts for hierarchical organization. Additionally, almost 400 new concepts covering food and environmental allergens and immune response triggers were added. The release also modernizes assessment tools, such as replacing the Hospice Item Set with the Hospice Outcomes and Patient Evaluation (HOPE) tool set starting in October 2025. Crucially, LOINC version 2.81 supports integration with HL7 FHIR, HIPAA-compliant data exchange, and initiatives related to the European Health Data Space, solidifying its role as a foundational standard for international interoperability. LOINC is used worldwide to enable the identification, exchange, and collection of clinical data across diverse health systems, containing over 108,000 concepts covering everything from specific lab values to imaging reports.
