BESTMED, a cloud-based medication management platform, and Best Practice Software, Australia's most widely used GP software, have announced an integration that marks a major step toward interoperability in aged care. The collaboration will link approximately half of Australia’s residential aged care homes with 70% of general practice (GP) facilities. The core functionality allows clinicians using Best Practice to log into BESTMED via single sign-on, providing instant access to up-to-date resident charts and medication lists. This eliminates duplicate data entry after aged care visits, streamlining workflows, improving prescribing accuracy, and saving valuable time for practitioners. Technically, the integration includes automated chart access and, critically, 'FHIR-based medication data exchange.' This allows prescribing data and clinical notes to flow directly from BESTMED into Best Practice. Once rolled out, the system will be available at scale to over 1,250 residential aged care homes nationwide. This partnership addresses a long-standing challenge in healthcare—the need for clinicians to manage patient data across multiple health verticals.
