Many projects utilizing FHIR are greenfield initiatives—a blank slate environment. This often leads management to assume that staffing can be done solely with new teams and developers, focusing on novel technologies and cloud infrastructure. However, this approach frequently results in a team lacking critical expertise: no domain knowledge, no healthcare data experience, no terminology expertise, no exposure to clinical workflows, no business domain experience, no privacy/compliance expertise, and critically, insufficient FHIR knowledge. Collectively, these gaps pose significant risks. The core problem is that these deficiencies often remain unseen until the project reaches an advanced demo stage, when more experienced professionals begin dissecting the system. Issues such as incorrect terminology, lack of auditing mechanisms, poor interoperability in custom data models, or flawed assumptions about HL7 message consistency are frequently exposed. While FHIR itself represents a modern technology, the field of healthcare and healthcare data is deeply historical. Therefore, while FHIR expertise is beneficial for new projects, it must be complemented by the deep knowledge and experience accumulated over years and decades within existing teams. This fundamental requirement remains constant.
