Conventional paper-based data transmission suffers from delays and incompleteness. Therefore, this study (CAEHR project) aimed to strengthen information linkage between hospitals and rehabilitation facilities. Based on structured interviews with medical experts, a web portal was built. Specifically, clinical and nursing data extracted from the primary hospital information system were mapped and transformed into FHIR format and openEHR format, respectively. After obtaining informed consent, this data is sent through a web portal hosted at university medical centers, which implements restricted external access on a Kubernetes system. Follow-up data, such as questionnaires and assessment outcomes, are returned from the rehabilitation facilities. The study has included 142 patients so far, demonstrating that a unified concept with comparable data flows can be applied even across university medical centers with different system architectures.
