Payer FHIR Interoperability Moves Into Production; Provider Readiness Still the Weak Link, Survey Finds

According to a flash survey by Black Book Research involving 118 payer IT leaders across 45 health plans, FHIR-based payer-provider data exchange is entering production in areas like Medicare Advantage. While payers show technical readiness (27% of health plans are already live on multiple FHIR workflows), the biggest blockers cited were contracting and data-use agreements (77%). The critical finding, however, concerns provider readiness. Only about one-third of provider organizations have begun testing or implementing FHIR endpoints for payer exchange. Furthermore, 41% of providers have not yet engaged with EHR vendors or clearinghouses, and only 17% have funded internal projects to support these workflows. The survey highlights three primary gaps: limited testing capacity within existing EHR environments; insufficient budgeting for integration and data quality in FY26; and incomplete operational planning to embed payer data into clinical workflows. Experts emphasize that sustained provider collaboration is now the determining factor for achieving production-scale FHIR interoperability, suggesting immediate actions like confirming FHIR support with vendors and forming cross-functional implementation teams are necessary. The report concludes that overall progress depends heavily on provider execution and EHR vendor enablement rather than just payer technical capability.
