Payer FHIR Interoperability Moves Into Production; Provider Readiness Still the Weak Link, Survey Finds

According to a Black Book Research flash survey of 118 payer IT leaders across 45 health plans, FHIR-based data exchange is advancing into production across various lines of business, such as Medicare Advantage. However, the study highlights a significant gap in readiness among provider organizations as of Q4 2025. Key findings show that 27% of health plans are already live with one or more FHIR workflows (e.g., member attribution, prior authorization). Furthermore, Medicare Advantage contracts are leading FHIR adoption compared to commercial lines. Despite payer-side technical progress, the primary bottlenecks identified at the provider level include testing limitations for connecting to payer APIs within existing EHR environments, insufficient budgeting for integration and data quality in FY26, and incomplete operational planning to embed payer data into clinical workflows. The survey emphasizes that sustained collaboration is crucial for achieving production-scale FHIR interoperability. Success depends heavily on the roadmaps of EHR and HIE vendors rather than just payer API readiness. To bridge these gaps, leaders are advised to form cross-functional teams and consider interim solutions like middleware integration platforms or 'FHIR-as-a-Service' adapters.
