This article describes the mechanism of adaptive forms, which are built upon the FHIR SDC IG. The system involves two key actors: a 'Clinical App' (which presents the form to the user) and an 'External Service' (which manages questions using algorithms or decision rules). Aidbox Forms supports the 'Clinical app side,' utilizing the `$next-question` operation. This allows the form to continuously request and generate the next question from the external service until the workflow is complete. This technology has broad real-world applications, including PROMs (Patient Reported Outcomes Measures), Prior Authorization processes, and Clinical Decision Support (CDS). For instance, it can trigger follow-up questions based on risk thresholds or dynamically request necessary documentation based on a diagnosis. Adaptive Forms serve as an intelligent foundation for data collection that goes beyond simple questionnaires, enabling the digitization of complex workflows—such as paper forms or prior authorization processes—into FHIR-compliant digital experiences.
