Elation Health faced the challenge of complying with the US 21st-Century Cures Act's FHIR mandate while maintaining its core EHR experience. Since rewriting the existing data structure was not viable, they needed a scalable solution to map legacy data to FHIR resources aligned with US Core profiles and enable secure interoperability. Edenlab built a backend utilizing Kodjin FHIR Server to allow Elation’s EHR to exchange data using the HL7 FHIR standard without rewriting the core system. Key technical features included a custom ETL pipeline for real-time data mapping, aligning with US Core profiles, and implementing SMART on FHIR functionality for secure third-party integration. Furthermore, the solution provided comprehensive support for ONC certification. This encompassed readiness assessments, defining testing scopes, internal validation of FHIR API endpoints, and coordination with third-party bodies like Inferno testing. Notably, they implemented an event-driven data streaming model that maintained high throughput (up to 1,160 RPS) while ensuring data integrity and security. This project allowed Elation Health to achieve regulatory compliance and realize future-proof interoperability, enabling real-time care coordination and advanced analytics in clinical settings.
