This national health insurance association is a federation of over 30 independent companies covering more than 100 million people. Previously, provider data was processed using an outdated file-based system that relied on weekly or monthly batch transfers, lacking scalability and real-time capability. This necessitated complex manual data conversion, which consumed significant time and increased risks regarding compliance with healthcare data governance standards. UST deployed a FHIR-based automated data exchange solution on AWS Cloud. The process utilized the Smile CDR server (designed with HL7 FHIR API standard) for processing and validating provider data, storing persistence in Amazon PostgreSQL RDS. Real-time data streaming was managed by Apache Kafka, and AWS Glue/PySpark automatically transformed multi-nested FHIR JSON datasets into flattened files. Furthermore, custom business rule validations were added to enhance data integrity, ensuring only accurate and compliant data is processed. For data access, the solution incorporated direct querying via Amazon S3 and AWS Athena, alongside a secure API exposure using Google Cloud's Apigee Gateway. This established an efficient and secure analytical platform, ensuring SLA adherence and enabling rapid adaptation to market changes.
