The Grupo Hospitalario Paravida in El Salvador, collaborating with the technology firm Illuminodes, is advancing a pilot implementation of a medical identity and health data management system utilizing the NOSTR protocol and the Bitcoin network. This decentralized model grants individual actors—such as patients or doctors—control over their digital identities via cryptographic keys, allowing them to decide what data is associated with their identity and who shares it. The initial phase focuses on digitizing internal operational processes within the hospital, such as appointment coordination and space management. The technical foundation uses NOSTR, a decentralized protocol where each actor controls their identity keys. Crucially, the system adopts HL7 FHIR (Fast Healthcare Interoperability Resources), an international standard for structuring clinical data, enabling interoperability between different systems. The core benefit of this decentralized model is that individuals maintain a personal 'digital folder' containing structured medical information. They can then selectively share only necessary fragments of data based on the context—for instance, sharing only specific lab results without exposing the entire medical history. Furthermore, cryptographic signatures are applied to the data, allowing verification to confirm if the information has been altered since it was issued. This initiative addresses the high cost associated with traditional centralized hospital management systems.
