Blockchain in E-Health: A New Model for Data Management Trust

Medical data is highly sensitive. Traditional centralized databases (resembling 1990s approaches) struggle with ensuring data integrity and tracking access history. While standards like FHIR solve 'format' issues, they leave unresolved the problems of 'trust' and 'ownership.' The author positions blockchain as an infrastructure that can mathematically establish trust between parties without a central intermediary. A key technical method described is storing highly sensitive medical records off-chain (in encrypted storage like IPFS), while only recording the document's cryptographic hash and metadata on the blockchain. This allows for immediate detection if the data has been tampered with, as the recorded hash would not match. Furthermore, concepts such as automated verification of insurance events via smart contracts, and 'Self-Sovereign Identity (SSI)'—where patients control and provide their own data—are introduced. Technically, permissioned blockchains like Hyperledger Fabric are deemed suitable for medical systems because they maintain high privacy while offering high throughput through participant authentication. The historical context of this shift is traced back to the concept of an 'immutable distributed ledger' established by Bitcoin in 2008. This was followed by its application in national systems (e.g., KSI Guardtime in Estonia) and accelerated by smart contract development after 2015, leading to current blockchain adoption in E-Health.
