This proposal extends existing interoperability requirements, specifically focusing on drug pre-authorization. Impacted payers are required to support APIs for electronic drug prior authorization. Furthermore, the Department of Health and Human Services (HHS) proposes adopting FHIR standards under HIPAA's Administrative Simplification provisions for related transactions. To streamline data exchange, three industry standards—NCPDP SCRIPT, F&B, and RTPB—are also required for pharmacy benefit coverage. This standardization aims to ensure technical uniformity across all medical services and drugs, enabling real-time data exchange and faster decision-making. Additionally, HHS proposes using the FHIR Da Vinci Clinical Data Exchange (CDex) IG for attachments accompanying prior authorization transactions, facilitating flexible sharing of relevant patient data across health IT systems.
