WEDI Launches FHIR API Testing Directory for CMS Interoperability Rule Compliance

The Workgroup for Electronic Data Interchange (WEDI) has launched a free API testing directory in preparation for the CMS Advancing Interoperability and Improving Prior Authorization Final Rule (CMS-0057-F), which mandates certain FHIR APIs by January 1, 2027. The rule requires entities to implement four specific APIs: Prior Authorization, Payer-to-Payer, Patient Access, and Provider Access. These APIs are designed to streamline prior authorization requests and responses, aiming to reduce burdens on providers, payers, and patients, with CMS estimating potential savings of $15 billion over the next 10 years. However, a survey conducted by WEDI in April 2025 revealed significant struggles among industry participants. Over half of providers and nearly 45% of payers reported not having started work on the required API implementations, citing issues such as insufficient funding, difficulties digitizing policies, and establishing cohesive interoperability strategies. To address this readiness gap, WEDI established the directory to facilitate testing between partners. The platform allows organizations to post their contact information, available APIs for testing, and website links. By encouraging real-time data exchange testing, WEDI aims to demonstrate the value of these APIs and accelerate compliance across the industry. Several major entities, including Wellmark and Mayo Clinic, have already joined the directory.
