Taiwan's Digital Health Transformation: Achieving Holistic Care with AI and FHIR

Taiwan leverages its advanced ICT industry foundation and National Health Insurance (NHI) data to advance digital healthcare under the 'Healthy Taiwan' vision. Central to this effort is the integration of electronic medical records across the country and ensuring interoperability by adopting the international standard, FHIR. Specifically, a framework known as “Kerangka 3-3-3” has been implemented, integrating three main health spaces, three data standards, and three national AI governance centers. This transition is shifting from treatment-centric to prevention/management-centric care, exemplified by services like the “Family Doctor Platform,” which uses AI predictive risk. Furthermore, the “My Health Bank” system integrates data from wearable devices, enabling citizens to actively participate in maintaining their health. FHIR standards are also utilized for exchanging Next-Generation Sequencing (NGS) data in cancer diagnosis, accelerating access to appropriate treatment. In the AI sector, 19 national medical AI centers have been established, ensuring responsible governance and clinical validation. Over 50 AI medical products have obtained regulatory approval. Taiwan is also developing federated learning—which allows model validation across institutions without moving sensitive data—and collaborating with ASEAN countries to build trusted international data exchange models. These efforts aim to expand healthcare services from hospitals into the entire community, realizing holistic care.
