As hospital digital infrastructure becomes central to 'Software-defined medicine,' AI agents are increasingly intervening in clinical processes like history collection and diagnosis support. This necessitates a shift beyond mere system management, elevating the role of the Chief Medical Information Officer (CMIO). The CMIO is defined as an expert who possesses deep clinical experience while leading IT strategy. Their mission is not just maintenance but transforming hospital processes and culture by linking emerging technologies like AI to solving real-world clinical problems. In areas such as data standardization (like FHIR) and EHR improvement, the CMIO acts as a 'standard setter,' ensuring data interoperability and quality. Taiwan's Ministry of Health and Welfare is supporting this role through the establishment of the 'Taiwan Smart Healthcare School.' This program offers a three-stage curriculum—from foundational knowledge to global perspectives and culminating in a Capstone Project—to train future healthcare architects. By maintaining a 'Physician First' approach at the clinical front, CMIOs act as cross-departmental diplomats. They use objective data and measurable outcomes to drive systemic change, making them crucial for advancing medical systems and ensuring hospital sustainability in the age of AI.
