Ministry of Health Reveals FHIR Box for Interoperability: Aiming for Nationwide Electronic Medical Record Sharing by 2027

The MOHW addressed the critical issue of 'information fragmentation' in Taiwanese healthcare, where medical records are scattered across over 40 different Hospital Information Systems (HIS) using inconsistent formats and coding. Instead of requiring a complete overhaul of existing HIS, the MOHW adopted a strategy to achieve interoperability through a centralized data platform. The solution is 'FHIR Box,' which functions as more than just an exchange system; it is a medical data interoperability OS. It integrates high-performance FHIR servers, SMART on FHIR application environments, and Clinical Decision Support (CDS) services. This open architecture allows various AI applications and services to be developed and deployed on a common foundation. FHIR Box automatically converts patient records from different hospital formats into international standards like FHIR. It also integrates with the Electronic Medical Record Exchange Center (EEC), enabling cross-hospital record retrieval simply by digital consent, eliminating the need for physical media or manual data handling. Furthermore, the MOHW is standardizing medical terminology nationwide using AI, successfully demonstrating conversions for major international coding systems: SNOMED CT, LOINC, and RxNorm. This involves utilizing LLMs to automatically process and correct records, ensuring high data compatibility. The deployment plan starts with all university-affiliated medical centers this year, expanding to regional and local hospitals by the end of 2027. The MOHW emphasizes that FHIR Box is intended to provide a standardized foundation for the entire industry, not replace existing HIS, thereby lowering the barrier for developers and institutions to build advanced AI applications.
