The case study addresses the challenge faced by an EHR partner needing to ingest and emit FHIR R4 across various healthcare networks (Patient Access, Prior Authorization, etc.). Previous integrations suffered from excessive development effort due to unique specifications and logic required for each network, compounded by deficiencies in terminology services and audit trails. To solve this, a strict FHIR R4 server was built on .NET, conforming to standards like US Core 6.1.0. The platform implemented 42 resource providers (including Patient, Observation, Consent) and provided native support for major terminologies such as LOINC and SNOMED CT. Furthermore, security and interoperability were ensured through SMART on FHIR v2.1 authentication and the TEFCA Common Agreement v2.0 QHIN handshake. The system incorporated PHI redaction and tamper-evident audit logging, resulting in a robust platform capable of handling complex industry requirements.
