Modern healthcare systems increasingly rely on standardized data for high-quality maternal services. This research utilized a Design Science Research (DSR) approach to develop an information model that implements FHIR interoperability in prenatal care. The model integrates diverse health concepts into a unified structure, addressing the specific needs of primary healthcare. The core focus was creating an information model mapped to the HL7 FHIR R4 standard, providing a holistic view of the patient’s pregnancy journey. By incorporating user stories from real-world primary care settings, the system ensures clinical relevance. Furthermore, adherence to international standards like ISO 13972:2022 validates its structural requirements for health informatics. Implementing such interoperable systems is a significant step toward digital health maturity in primary care. It bridges data sharing gaps among complementary healthcare professionals, enabling multidisciplinary teams (doctors, nurses, midwives) to provide highly personalized and coordinated patient care, thereby significantly reducing the risk of fragmented care.
