AJ FHIR Platform: Comprehensive Open-Source Infrastructure for Healthcare Interoperability

This platform consists of nine components, including support for SMART on FHIR v2.2 and HL7v2, built on an open-source FHIR foundation running on Java 21. The core functionalities include a secure authentication server (Auth Server) enforcing PKCE S256 for login management, consent management (Consent Manager) enabling patient-driven access control compliant with GDPR and HIPAA, and a gateway (AJ Gateway) from HL7v2 to FHIR. This ensures that when apps are used in clinical settings, only necessary data is retrieved based on the principle of least privilege, and all access events are recorded in an audit log. The system achieves high security and compliance by adhering to multiple international standards such as IHE ATNA and PDQm. It provides a complete technical picture from browser to database, built upon SMART App Launch v2.2, FHIR R4, and various IHE profiles.
