This document is a technical reference detailing an 'OperationDefinition' called 'Transform from PHMR,' provided by eHealth Infrastructure. The specification aims to convert documents in the DK-HL7 Personal Health Monitoring Report (PHMR) XML format into FHIR Observation resources. Specifically, it accepts a DocumentReference containing the PHMR XML as input and generates a Bundle that includes one or more FHIR Observation resources and a Composition resource (ehealth-composition profile). This mechanism provides an essential foundation for exchanging personal health monitoring data—which is often collected in various formats—in a standardized manner compliant with the international FHIR standard. This transformation process allows local health data, which may have different structures or formats, to be mapped into the unified data model of FHIR Observation. This significantly contributes to improving interoperability within Japanese healthcare IT systems. By doing so, it becomes possible to ingest diverse biometric data obtained from sources like wearable devices in a standardized electronic record format.
