This article details the introduction of CLARYS by Espeir. This solution is designed not only to process structured data but also to extract clinical concepts from vast amounts of unstructured document data, providing a comprehensive visualization of the entire patient journey. CLARYS is built on a FHIR foundation and supports integration with international reference standards such as SNOMED CT, LOINC, ICD 10/11, and RxNorm. This allows it to consolidate data from various systems and formats (e.g., operative reports, prescriptions, observations) into an 'medical memory'—providing information that is clinically meaningful and usable, rather than just a collection of raw data. The approach offers significant benefits: it can be implemented without modifying existing Electronic Health Record (EHR) systems; operationally, it reduces search time; and strategically, it ensures application independence. Furthermore, it emphasizes data security through traceable audit trails and guarantees data continuity. Given that integrating and standardizing data from diverse sources is a major challenge in Japanese healthcare, this solution holds high relevance for the Japanese medical field.
